We are very excited that you have attended the required 3 breakfast meetings and you are now applying for Club Membership. Please complete the form below so we can further process your Application as a Prospective Member.
Our Club has moved to a paperless application process that is integrated with our website database. Upon your induction as a Rotarian, you will have a unique login to our Member's Only exclusive section of the website.
The form is divided into to sections; Public and Private. The Public information is included in the Member Directory section of the web site Rotarian Members Directory. The Private section is only used in the Member's Only secure section of the site. At no time is any of your personal information provide to any 3rd party out site of Rotary.
After completion of and submitting the form below, the Membership Committee will be notified and will follow-up with you. Your Application is submitted to the Board of Directors and then the general Club Membership for final review.
By submitting this application the Prospective new member confirms they have read and agree to Carbondale Rotary Member's Responsibilities.
By submitting this application, I grant permission to present my name and information contained in this Application Form"Statement" to the Carbondale Rotary Club membership. I agree to the Financial and Time Commitment and after attending 3 meetings, if asked, I will make my final decision to become a Rotarian within 24 hours by notifying the Membership Chairman.
